The Cost of Getting This Wrong
Maryland's full practice framework means freedom — but it also means that every compliance failure lands squarely on you. There is no supervisory physician whose oversight serves as a buffer. These are the mistakes Maryland CRNPs make most often, and the ones with the most serious consequences.
Prescribing controlled substances without a Maryland Drug Control Number. Maryland requires two separate registrations before a single controlled substance prescription is written — a federal DEA registration and a Maryland Drug Control Number issued by the Maryland Board of Nursing. Most CRNPs know about the DEA requirement. Many miss or delay the state Drug Control Number. Prescribing on DEA alone, without the Maryland number, is a state law violation that can result in MBON disciplinary action, prescription invalidation, and pharmacy complaints.
Failing to register with and query the PDMP. Since July 1, 2018, Maryland has mandated PDMP registration and active querying as a legal condition of controlled substance prescribing — not a best practice, a legal requirement. Before initiating any new course of opioid or benzodiazepine treatment, you must query the CRISP portal and document that query in the patient record. If treatment extends beyond 90 days, a new query is required at least every 90 days. Failure to comply is a prescribing violation regardless of your clinical rationale.
The 18-Month Mentorship: What It Actually Requires
Maryland's mentorship framework is deliberately lighter than the supervised practice periods in states like Maine (24 months) or similar transition-period states. Here is precisely what it does — and does not — require.
Who it applies to. Only CRNPs applying for initial Maryland certification who have never held NP certification in Maryland or any other state. If you are an experienced NP relocating from another state, the mentorship requirement does not apply to you. Maryland recognizes your existing experience and grants full independent authority from the date of licensure.
Who qualifies as a mentor. A licensed Maryland physician, or an experienced CRNP who is a certified nurse practitioner and has at least three years of clinical practice experience. This is a significant distinction from states that restrict mentors to physicians only — in Maryland, an established CRNP colleague can fulfill the mentorship role.
What the mentor's obligation actually is. The mentor must be available for advice, consultation, and collaboration as needed during the 18-month period. That is the full legal obligation. There are no requirements for scheduled meetings, chart co-signatures, minimum consultation frequency, geographic proximity, or written board-filed agreements, and routine consultation and referral with other health care providers remains part of ordinary team-based practice. The mentor does not assume regulatory responsibility for the new NP's clinical decisions.
What is not required. No written agreement needs to be filed with the Maryland Board of Nursing. No periodic reports. No completion letter. No Board approval is required before the mentorship starts, and the NP does not need to submit a separate pre-approval request. The mentorship is identified and maintained at the practitioner level — the Board does not track or confirm the relationship the way Maine's Board does with its supervisory registration. This places the compliance burden entirely on the CRNP to maintain an actual, accessible mentor relationship throughout the 18 months.
CPA Content Requirements
Maryland requires no standardized CPA form for CRNPs, and experienced practitioners need no agreement at all. Even where Maryland law does not require a CPA, collaboration standards still matter for patient safety, as reflected in American Academy of Nurse Practitioners guidance. For new CRNPs establishing a mentorship arrangement, and for any CRNP entering a voluntary collaboration for credentialing or institutional purposes, a well-structured agreement should include:
Identities and Credentials — Full legal names, Maryland license or certification numbers, DEA registration numbers, Maryland Drug Control Numbers, and national certification details for both parties, with physician participants identified as licensed physicians
Scope of Arrangement — Clear definition of whether the agreement is a mentorship, a voluntary collaboration, or an institutional credentialing requirement — and what clinical scope it covers
Availability and Consultation Protocol — How the mentor or collaborating physician will be reachable, expected response times for non-urgent and urgent consultations, and preferred communication channels
Documentation Standard — How consultations will be logged, including date, topic, format, and outcome — critical if the arrangement is ever reviewed by a credentialing body or the Board
PDMP Acknowledgment — Confirmation that the CRNP holds active PDMP registration and complies with Maryland's mandatory query requirements
Term and Duration — Start date, end date (for mentorship: 18 months from initial application receipt), and renewal or continuation terms for voluntary arrangements
Termination Provisions — Minimum notice period for either party, and a plan for identifying a replacement mentor if the arrangement ends before the 18 months are complete
Signatures and Dating — Original dated signatures from both parties; retain copies independently since no board filing is required
Some practices, clinics, or insurance plans may still ask the office to keep a written attestation in the credentialing file, even though that is a policy requirement rather than a Maryland legal filing.
Controlled Substance Rules and Prescriptive Authority
Maryland CRNPs hold full, independent authority to prescribe Schedules II through V controlled substances. Two registrations are both mandatory before any controlled substance prescriptions are written.
Federal DEA Registration — Apply through deadiversion.usdoj.gov using Form 224 for mid-level practitioners. Allow 4–6 weeks for new applications. The DEA registration is address-specific and must be updated with any practice location change. Renews every three years at a current cost of $888.
Maryland Drug Control Number — Issued by the Maryland Board of Nursing. This is Maryland's state-level controlled substance registration, separate from and in addition to the DEA. You cannot legally prescribe controlled substances in Maryland on DEA registration alone. Apply through the MBON and track the renewal cycle independently from your DEA renewal.
Maryland PDMP — Mandatory Registration and Use — Maryland's Prescription Drug Monitoring Program is managed through the CRISP portal (crisphealth.org). Registration is a prerequisite for obtaining or renewing your Maryland Drug Control Number — you cannot have one without the other. Active querying is legally mandated in two specific circumstances: before initiating any new course of opioid or benzodiazepine treatment, and at least every 90 days when such treatment extends beyond 90 days. Document every query in the patient's medical record. You may delegate the act of pulling PDMP data to a qualified staff member, but reviewing and assessing it before prescribing remains your legal responsibility.
Physician Ratio & Geography
Maryland imposes no ratio caps on CRNP mentorship or voluntary collaboration arrangements. A physician or experienced CRNP mentor can support any number of new NPs simultaneously — the state places no ceiling on it.
There are also no geographic proximity requirements. Your mentor or any voluntary collaborating physician can be located anywhere in Maryland. Teleconference-based consultation fully satisfies the mentorship availability requirement. For Maryland CRNPs practicing in rural Western Maryland, the Eastern Shore, or other areas with thinner physician density, this matters — you are not forced into a narrow local pool.
Note that this is distinctly different from the rules governing Physician Assistants in Maryland. PAs are subject to a statutory cap of four PAs per supervising physician and must maintain a formal board-filed supervision agreement. Maryland CRNPs operate under an entirely different — and significantly less restrictive — framework.
Ongoing Compliance
Full practice authority in Maryland means full accountability. Key ongoing state and federal compliance regulations after licensure:
DEA registration — Renews every three years; tied to your practice address. The DEA does not proactively send expiration reminders. Build renewal reminders at least 60 days before expiration
Maryland Drug Control Number — Track renewal independently from DEA. Lapse in either registration means you cannot legally prescribe controlled substances under Maryland or federal law
PDMP registration — Must be active as a condition of holding your Maryland Drug Control Number. Log into the CRISP portal periodically to confirm registration status
PDMP mandatory queries — Before every new opioid or benzodiazepine course; every 90 days for ongoing treatment. Document every query in the chart
National certification — Must remain active throughout the CRNP license period. Notify the MBON immediately if certification lapses; cease practicing as a CRNP until it is restored
CE requirements — Maryland RN license renewal requires 30 contact hours per two-year cycle. Your national certifying body (ANCC, AANP, PNCB) runs a separate CE cycle for credential renewal. Track both independently
Scope of practice — Board investigations of independent practitioners focus on whether clinical decisions fell within the CRNP's certified population focus. Independent practice still operates within broader standards shared across Maryland healthcare providers. Practicing outside your certification area is a violation that full practice authority does not excuse
Telemedicine in Maryland
Maryland extends its full practice authority framework equally to telehealth settings — no additional physician oversight or special telehealth certification is required for Maryland CRNPs practicing virtually.
Payment parity — Maryland law requires private insurers to reimburse telehealth services at the same rate as equivalent in-person services. This is one of the stronger telehealth parity laws in the country and is a meaningful financial consideration for Maryland CRNPs building telehealth-based practices.
Prescribing via telehealth — Maryland does not impose prescribing restrictions specific to telehealth encounters. The same PDMP query obligations that apply in-person apply equally to telehealth encounters. A clinical evaluation — in-person or virtual — is required before writing a covered prescription; document it thoroughly.
Controlled substances via telehealth — Federal Ryan Haight Act rules govern controlled substance prescribing via telehealth.
